Event Submission Form

All individuals or organizations that would like to organize a public event in the HUB Third Avenue BID must submit this form in addition to following all requirements of the Street Activity Permit Office (SAPO). Submission is not a guarantee of approval by the HUB. Please complete this form accurately. Please allow at least two weeks for HUB staff to review your submission.
